Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in New Haven, CT. The role focuses on planning, implementing, and documenting occupational therapy interventions to support resident care.

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention for each patient.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and team members.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to treatment plan and document/bill services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.
