Job summary

Evolve Therapy Services, LLC in Torrington, CT seeks aCertified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. Full- and part-time or PRN roles are available to support residents and the care team, delivering high-quality OT under supervision.

The ideal candidate holds an Associate’s degree in Occupational Therapy, an active OTA license, and is committed to ongoing professional development, with emphasis on evaluation, treatment planning, documentation, and teamwork

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment.
• Perform treatment plans and progress toward goals per plan of care.
• Interpret and communicate evaluation findings to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy per treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.
