Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in New Haven, CT. Full Time, Part Time, or PRN positions available.

The OTA will plan, organize, and direct occupational therapy services in a facility that adheres to federal, state, and local standards to ensure high-quality resident care. Responsibilities include screening residents, implementing treatment plans, communicating evaluations, scheduling therapy, and ensuring CMS

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine interventions.
• Develop and follow treatment plans with supervision.
• Communicate findings to residents, families, and team.
• Schedule resident therapy with team members.
• Document and bill occupational therapy services per CMS standards.
• Perform duties to effectively treat patients as assigned.
