Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in New Haven, CT. The role supports therapy services across full-time, part-time, or PRN positions to deliver high-quality resident care.

Responsibilities include screening, developing plans, coordinating with the interdisciplinary team, and ensuring proper documentation and billing in accordance with CMS guidelines.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Perform treatment plans and follow short- and long-term goals.
• Communicate evaluation findings and recommendations to residents, families, and team.
