Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in New Haven, CT. The role supports Full Time, Part Time, or PRN schedules and focuses on delivering high-quality occupational therapy services.

Responsibilities include screening residents, implementing treatment plans, coordinating with the interdisciplinary team, and maintaining CMS-compliant documentation to ensure optimal resident care.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Perform treatment plans and follow established goals.
• Interpret and communicate evaluation findings to residents and team.
• Establish resident therapy schedules with other team members.
• Document and bill occupational therapy services per plan.
• Perform other duties to effectively treat patients.
• Comply with CMS documentation standards daily.
