Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) per IDFPR.
• Basic Life Support (BLS) within 30 days - AHA.
• Continuing education for stroke care as required.

Responsibilities

• Performs patient transfers with assistance and maintains safe body mechanics.
• Educates patients and families using effective teaching methods.
• Designs and adjusts a plan of care based on patient response and evidence.
• Evaluates findings to determine PT diagnoses and outcomes.
• Performs examinations with reliable methods and documents results.
• Completes daily documentation and charging in therapy EMR.
• Uses therapeutic modalities as appropriate and communicates progress with providers.
