Job summary

LiveWell Homecare Agency is looking for a licensed Physical Therapist to provide medical therapy aimed at reducing pain and restoring function. You will design individualized treatment plans, supervise staff, and document progress while collaborating with a care team in a homecare setting.

The role requires a DPT and active PT licensure, with BLS and pelvic health certifications preferred. Expect involvement in training and ongoing professional development within a patient-centric program.

Qualifications

• Requires a Doctorate in Physical Therapy (DPT) and active Physical Therapist licensure.
• BLS certification and pelvic health certification are required or preferred.
• Experience expectations and additional certifications are advantageous.

Responsibilities

• Plan and conduct therapy programs including exercises, manual techniques, heat, water, light, and electrical treatments.
• Perform diagnostic and functional tests to assess patient needs.
• Guide patients through active and passive exercises, gait training, and use of assistive devices.
• Manage equipment such as ultrasound and diathermy machines.
• Treat wounds and instruct on postural control and therapeutic exercises.
• Document patient progress and outcomes.
• Supervise support staff and assist with training students and staff.
