Job summary

LiveWell Homecare Agency, LLC is seeking a licensed Physical Therapist to deliver medically prescribed therapy aimed at reducing pain, restoring function, and preventing disability after illness or injury. You will conduct evaluations, design individualized therapy programs, guide exercises, supervise staff when needed, and educate patients on mobility aids and self-management.

A DPT is mandatory; BLS certification required; pelvic health certification preferred and should be obtained within 90

Qualifications

• Must hold a current Physical Therapist license.
• BLS certification required; pelvic health certification preferred.
• Post-2015, a Doctorate in Physical Therapy (DPT) is mandatory.
• Experience in therapy and patient care preferred.

Responsibilities

• Conduct evaluations and develop individualized therapy programs.
• Guide patients through exercises and education on mobility aids.
• Maintain progress records and collaborate with rehab team.
• May supervise staff and train students in therapy techniques.
