Job summary

Azza HealthCare Agency is seeking a licensed Physical Therapist to provide medically prescribed therapy, relieve pain, and restore function. You will evaluate patients, create and adjust treatment plans, and supervise therapy sessions as needed.

Qualifications

Job description

Role Overview:

The Physical Therapist provides medically prescribed therapy to relieve pain, restore function, and prevent disability after disease or injury.

Key Responsibilities:

Conducts evaluations, develops treatment plans involving exercises, manual therapy, and modalities like heat and ultrasound. Guides patients in active/passive exercises, muscle re-education, gait, and functional training. Uses specialized equipment and directs patient care, including assistive devices. Performs patient assessments, monitors progress, and maintains records. May supervise staff and participate in training students or staff in therapy techniques.

Qualifications & Skills:

Requires a Doctorate of Physical Therapy (post-2015) and licensure as a Physical Therapist. Must hold certifications such as Basic Life Support (BLS), and preferably in Pelvic Health. Experience expectations include a minimum number of years and specific types of experience.

Additional Requirements:

Certification in pelvic health is preferred; other certifications may be required. Must obtain specific certifications within 90 days if not already held.
