Job summary

Live Well Homecare seeks a licensed Physical Therapist to provide medically prescribed therapy, evaluate and restore function, and prevent disability. You will conduct patient assessments, develop personalized treatment plans with exercises, manual therapy, and modalities, and supervise therapists and aides.

The role requires a DPT, active PT license, and BLS. Pelvic health certification is preferred and must be pursued within 90 days of hire.

The Physical Therapist provides medically prescribed therapy to alleviate pain, restore function, evaluate, and prevent disability caused by disease, injury, or loss of a body part. They develop personalized therapy programs using techniques such as exercise, massage, manual therapy, heat, water, light, and electrical modalities as directed by physicians. Key responsibilities include conducting diagnostic and functional ability tests, guiding active and passive exercises, muscle reeducation, gait training, and utilizing equipment like ultrasound and diathermy machines. They treat wounds, instruct patients on the use of mobility aids and orthotic devices, and perform patient evaluations with progress documentation. Supervising assistants and aides, as well as training students and staff in therapy techniques, may also be part of the role.

Qualifications & Skills

Education

A Doctorate of Physical Therapy (DPT) is required after a specified date. A high school diploma is the minimum educational requirement.

Experience

Experience requirements include a specified number of years in relevant settings, with preferred experience types indicated.

Certifications & Licensures

Must hold a valid license as a Physical Therapist, Basic Life Support (BLS) certification, and relevant certifications such as pelvic health. Additional certifications like pelvic health are preferred.
