Holyoke Medical Center is seeking a Nurse Navigator in Nursing for a full-time day shift. The role coordinates care before surgery, during the postoperative period, and after discharge, collaborating with providers, patients, and families to ensure timely, high-quality care.

Responsibilities include educating patients, arranging pre-procedure evaluations, and developing individualized care plans while maintaining care standards and communication across teams.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license within the Commonwealth of Massachusetts.
• Three to five years of clinical related job experience, case management experience preferred.

Responsibilities

• Provide comprehensive patient education regarding procedures, treatment plans, and the continuum of care.
• Coordinate pre-procedure evaluations, testing, appointments, and clearances to support timely care.
• Assess patient needs and develop individualized care plans, including identifying potential risks or barriers.
• Serve as a liaison among patients, families, providers, and multidisciplinary healthcare teams.
• Monitor compliance with established care processes, protocols, and treatment plans.
• Identify and communicate issues that may affect the timing or effectiveness of patient care.
• Develop educational resources and tools to promote patient engagement and self-management.
• Collect, analyze, and report patient outcomes and quality data to support continuous improvement.
• Coordinate discharge planning, follow-up care, and post-discharge services to promote safe transitions and reduce readmissions.
• Monitor patient and family satisfaction, participate in quality improvement initiatives, and support the development of care management policies, procedures, and electronic tools.
