The role supports patient care navigation from pre-procedure scheduling through post-operative care, coordinating with physicians and the multidisciplinary team to ensure timely access to care and adherence to treatment plans.

The position requires RN licensure in Alabama and an ASN or BSN degree, with experience in acute care and orthopedic or neurological specialties.

Qualifications

• Education/Training: ASN and BSN nursing degrees.
• Licensure/Certification: Alabama RN license and current BLS/healthcare provider certification.
• Experience: clinical experience in acute care; Ortho/Neuro experience.

Responsibilities

• Coordinates patient care through the continuum. Assists with timely access to care, coordinates/facilitates appointments and services.
• Assesses patients’ current and future needs, provides access to resources, and makes referrals as needed.
• Provides education to patients and families related to the healthcare system, team roles, plan of care, and available resources.
• Maintains collaboration with physicians and multidisciplinary team to ensure seamless care and coordination.
• Documents patient interactions and communicates navigation activities to physicians and leadership.
