Job summary

Baptist Health Brookwood Hospital in Birmingham, Alabama, is seeking an Orthopedic Nurse Navigator to guide patients through the care continuum from pre-procedure scheduling to post-operative follow-up. The Navigator coordinates with physicians, nursing staff and multidisciplinary teams to ensure timely access to care and consistent, patient-centered decision making.

Education requirements include ASN or BSN and an Alabama RN license with current BLS.

Qualifications

• ASN or BSN required to practice as RN in Alabama.
• Active Alabama RN license with current BLS certification.
• Acute care experience, with orthopedic or neurosurgical exposure preferred.
• Proven ability to educate patients and coordinate care across multiple teams.

Responsibilities

• Coordinate patient care through the care continuum and assist with timely access to services.
• Assess patient needs and make appropriate referrals to internal/external resources.
• Provide patient and family education regarding plans of care and available resources.
• Collaborate with physicians and multidisciplinary team to ensure seamless care coordination.
• Document patient interactions and track program metrics as required.

Serving the Homewood community, Baptist Health Brookwood Hospital is a 595-bed acute-care hospital located in Birmingham, Alabama. The facility offers a full complement of services, including emergency care, cardiovascular care, bariatrics, orthopedics and rehabilitation, and surgical services. At Baptist Health Brookwood the Orthopedic Nurse Navigator enhances the patient's experience by providing seamless navigation from pre-procedure scheduling and throughout the care continuum. The Navigator collaborates with all members of the interdisciplinary team to coordinate and consistently manage patient care by serving as a point of contact for patients and families. Systematically and continually performs the functions of assessing, planning, implementing, and evaluating care according to the nursing process.

Responsibilities

• Coordinates patient care through the continuum. Assists with timely access to care, coordinates/facilitates appointments and services, and encourages adherence to treatment plans, protocols, and follow up care.


• Assesses patients’ current and future needs, provides access to resources (internal and external), and makes appropriate referrals as needed.


• Provides education and information to patients and families related to the healthcare system, multidisciplinary team member roles, plan of care, and available resources.


• Establishes and maintains close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and care coordination.


• Tracks determined program metrics and quality indicators. Documents patient interactions and progression, and communicates navigation activities to physicians, the care team, and leadership.


• Collaborates with the clinical team to obtain or develop patient and family educational materials and resources.


• Demonstrates knowledge, skill, and coordination to provide nursing care and guidance to patients from screening to post-operative care and follow up.


• Demonstrates effective communication with patients, families, peers, members of the multidisciplinary healthcare team, and representatives of community organizations and resources.


• Maintains reasonably regular, punctual attendance consistent with Baptist Health policies, the ADA, FMLA and other federal, state and local standards.


• Maintains compliance with all Baptist Health policies and procedures.


• Actively participates in multidisciplinary planning conferences.


• Attends staff development in-services, department meetings, etc.


• Establishes and maintains professional role boundaries with patients, families, and the multidisciplinary care team.


• Serves as an advocate for patients and families.
