Job summary

Revere Health in Provo is seeking an Orthopedic Patient Transition Coordinator to support patients during transitions within our orthopedic clinic. You will guide patients to appropriate providers, ensure continuity of care, and assist with office transition tasks in a fast-paced environment.

The role emphasizes compassionate patient support, knowledge of orthopedic care pathways, and proficiency with EHR systems. Some travel to other Utah clinics may be required.

Qualifications

• Experience in orthopedic or healthcare settings.
• Strong communication and patient-centered approach.
• Proficient with EHR systems and MS Office.
• Ability to navigate insurance networks and referrals.
• Must be a current employee of Revere Health’s Provo Orthopedic group.

Responsibilities

• Serve as the primary office contact for patients seeking orthopedic follow-up or continued care.
• Assist patients in understanding their care options, including in-network and external resources.
• Guide patients with complex questions regarding their care and follow up options.
• Work closely with internal departments to ensure smooth handoffs.
• Maintain up-to-date knowledge of available providers and services in the region.
• Ensure timely communication with patients via phone, email, or other channels.
• Document all interactions in the EHR.
• Escalate patient concerns to leadership or care coordination teams as appropriate.
• Maintain confidentiality and HIPAA compliance.
• Take inventory of assets, DME, medications, and other supplies.
• Assist with closure of outstanding vendor accounts.
• Assist with other office transition tasks as necessary.
• Travel to other Revere Health clinics within Utah County as needed.
