Orthopaedic Associates of Michigan (OAM) is seeking a Patient Navigator in Grand Rapids, MI. You will serve as a key contact, guiding patients through their orthopedic care journey and coordinating with the Physical Therapy clinic to ensure efficient, personalized care.

Qualifications

• 1-2 years of customer service experience.
• High School Diploma or GED.
• Proficiency in MS Office.
• Preferred: 1-2 years in a healthcare setting.

Responsabilités

• Handle patient interactions with care; anticipate questions and respond thoroughly.
• Provide timely access to ancillary services by reviewing internal referrals.
• Assist patients and families in choosing appropriate services for continuum of care.
• Work with clinical and operational leaders to identify patient access issues.
• Monitor and update Physical Therapy schedules and laser therapy appointments.
• Ensure confidentiality of patient information in accordance with HIPAA.
