Job summary

Acadiana CARES, in partnership with the Louisiana Office of Public Health-STI/HIV/Hepatitis Program, is recruiting for a Linkage to Cure/Treatment Coordinator in Lafayette. The role focuses on connecting patients to HIV/HCV care and supportive services, with emphasis on vulnerable populations and overdose prevention strategies.

Candidates must have a Bachelor’s degree in a social science or related field and at least two years of social service experience.

Qualifications

• Bachelor’s degree in a social science or human related field.
• Two years of social service experience working with vulnerable populations and/or experience in overdose and infection prevention strategies and community engagement.

Responsibilities

• Provide treatment adherence services for Hepatitis C Virus (HCV) and connect individuals to medical care and community-based services.
• Provide treatment adherence services for HCV/HIV and connect individuals to medical care and community-based services.
• Provide medical information and navigation services to individuals with barriers to cure.
