STI HIV Hepatitis Program in Louisiana seeks two Linkage to Care Coordinators in the New Orleans region. One oversees Regions 1 & 9 (New Orleans, Covington, Hammond, Northshore); the other oversees Regions 1 & 3 (New Orleans, Houma, Thibodeaux).

You will connect newly diagnosed persons with HIV into medical care and community-based supportive services, support adherence for those out of care or with high viral load, and make referrals to resources to overcome barriers faced by people who use

Qualifications

• Bachelor’s degree in a social science or related field.
• Two years of social service experience with vulnerable populations.
• Knowledge of overdose prevention and infection prevention strategies.

Responsabilités

• Connect newly diagnosed persons with HIV into medical care and community-based services.
• Provide treatment adherence services to re-engage those out of care.
• Support adherence for PWH with elevated viral load.
• Refer to community resources to address barriers for PWH.
