STI HIV Hepatitis Program in Louisiana seeks two Linkage to Care Coordinators in the New Orleans region. One oversees Regions 1 & 9 (New Orleans, Covington, Hammond, Northshore); the other oversees Regions 1 & 3 (New Orleans, Houma, Thibodeaux).

You will connect newly diagnosed persons with HIV into medical care and community-based supportive services, support adherence for those out of care or with high viral load, and make referrals to resources to overcome barriers faced by people who use

Formación

• Bachelor’s degree in a social science or related field.
• Two years of social service experience with vulnerable populations.
• Knowledge of overdose prevention and infection prevention strategies.

Position overview

Linkage to Care Coordinators (LCC) (2) New Orleans/Hammond New Orleans/Houma Acadiana

Responsibilities

• connect newly diagnosed persons with HIV (PWH) into medical care and community-based supportive services
• provide treatment adherence services to PWH who have fallen out of care to re-engage them with routine HIV medical care
• provide treatment adherence services to PWH who have an elevated viral load
• make referrals to community resources to assist PWH overcome systemic and structural barriers

Positions

Two (2) positions available; One (1) overseeing Regions 1 & 9 (New Orleans, Covington, Hammond, Northshore); One (1) overseeing Regions 1 & 3 (New Orleans, Houma, Thibodeaux).

Applicant qualifications

Applicants must have excellent skills in community engagement and working with vulnerable populations and excellent facilitation skills. A general knowledge of HIV, HCV, and/or STIs, case management, and health education and risk reduction are needed. Additional requirements include experience working with people who use drugs (PWUD), medical care and treatment management, and general knowledge of overdose and infection prevention strategies.

Required qualifications

• Qualified candidates must have a Bachelor’s degree in a social science or human related field.
• Two (2) years of social service experience working with vulnerable populations and/or experience in overdose and infection prevention strategies and community engagement.
