Qualifications

• Experience coordinating care across outpatient and surgical pathways is desirable.

Responsibilities

• Provide patient care and care coordination in ophthalmology.
• Coordinate preoperative scheduling and postoperative follow-up.
• Assess, plan, implement, and evaluate age-appropriate care for patients.
• Advocate for patients and coordinate referrals to social services and ancillary teams.
• Utilize data tools to track patients through the preoperative process.
