Job summary

The Cincinnati VA Medical Center is seeking a Registered Nurse Case Manager for Ophthalmology to deliver fundamental, knowledge-based care to assigned clients. You will assess, plan, implement, and evaluate care focusing on education, self-management, and patient satisfaction across the continuum of care.

Qualified candidates will possess RN licensure, an approved nursing education program, and demonstrated ability to coordinate multi-disciplinary care for ophthalmology patients.

Qualifications

• Nursing degree from an approved program meeting VA standards.
• License to practice as a registered nurse in the United States.
• Experience in ophthalmology or case management preferred.

Responsibilities

• Provide patient care, care coordination, and health education.
• Coordinate pre-operative and post-operative processes and referrals.
• Demonstrate leadership in managing complex patient cases and documentation.
