Job summary

The VA Northeast Ohio Healthcare System is seeking a Registered Nurse Telehealth Case Manager within Community Outpatient Services to provide evidence-based care, care coordination, and discharge planning for patients in inpatient and outpatient settings.

Responsibilities include coordinating care with Veterans, families, and caregivers, guiding non-face-to-face telehealth visits, and collaborating with PACT teams to optimize outcomes. Travel requirements and telehealth education are included.

Qualifications

• Experience in case management and telehealth coordination.
• Registered Nurse with active licensure in the United States.
• Knowledge of VA telehealth practices and patient education strategies.

Responsibilities

• Provides patient management, care coordination, and discharge planning for inpatient and outpatient settings.
• Assists Veterans, family members, and caregivers in obtaining appropriate options and services.
• Coordinates care with PACT and specialty providers to improve disease management and resource use.
• Promotes non-face-to-face telehealth encounters and education for patients and caregivers.
• Ensures safe administration of medications and adherence to clinical policies.
• Supports telehealth program outreach and education to care teams and patients.
