Job summary

The Cincinnati VAMC is seeking a Registered Nurse Case Manager - Ophthalmology to deliver fundamental knowledge-based care to assigned patients. This role coordinates care across the continuum with emphasis on patient education, self-management, and customer satisfaction.

The duties include assessment, planning, implementation and evaluation of care, and collaboration with multi-disciplinary teams to support outcomes.

Qualifications

• English proficiency required.
• Graduate of a nursing program approved by ACEN or CCNE.
• Current, full, active, and unrestricted RN license in the United States (or DC).
• Foreign nursing education not creditable unless formal equivalency validation.
