Qualifications
- Associate's degree in nursing and active RN license.

- Experience with EMR/EHR documentation and patient care planning.
Responsibilities
- Assess patient conditions and develop care plans with the team.
- Administer medications and monitor progress.
- Document care activities and ensure accurate records.
- Educate patients and families on health conditions and post-discharge care.
- Collaborate with the healthcare team to ensure safe, coordinated care.
