Job summary

The Christ Hospital is seeking a Registered Nurse to provide holistic patient care within a collaborative care model. The RN will assess, plan, implement, and evaluate nursing care in alignment with ANA code of ethics and Ohio Nurse Practice Act, ensuring patient and family partnership throughout care delivery.

Responsibilities cover assessment, diagnosis and planning, implementation, evaluation, and documentation with emphasis on safety, education, and coordination with the healthcare team.

Qualifications

• Assessment demonstrates holistic patient assessment including physical, psychosocial, spiritual, and educational needs.
• Diagnosis and planning uses collected data to set measurable goals with patient/family involvement and SBAR communication.
• Implementation provides evidence-based care, follows orders, educates patients/families, and maintains safety practices.
• Evaluation continuously reviews plan effectiveness, adjusts care, and ensures continuity during handoffs and transfers.
• Documentation accurately records care plans and patient responses in the EMR per policy.

Responsibilities

• Assessment: perform holistic patient assessments and identify needs.
• Diagnosis and Planning: prioritize problems, set goals with patient/family, use SBAR.
• Implementation: deliver care per orders, educate patient/family, ensure safety.
• Evaluation: monitor outcomes, adjust plan, ensure continuity across shifts.
• Documentation: record plan of care and outcomes in EMR timely.
