Qualifications

• Two years in a primary care setting or ambulatory clinic focused on patient navigation.
• Experience with care transitions desired.
• Must hold an unrestricted Texas RN license.

Responsibilities

• Engage MAP patients during hospital admissions.
• Coordinate care plans with patients and families.
• Provide patient education on medication and follow-up care.
