Qualifications

• Registered Nurse license (RN) required.
• Bachelor's or Master's degree, or RN Diploma accepted.
• 3 years of experience required.

Responsibilities

• Guide patients and families through care planning and education.
• Coordinate services and develop individualized treatment and discharge plans.
• Collaborate with physicians, nurses, and care teams to ensure safe transitions and high-quality care.
• Complete psychosocial assessments and document findings for care teams.
