Job summary

CRCD in Los Angeles seeks an ICMS Case Manager to provide coordinated case management and supportive services for formerly homeless individuals with special needs. Services are delivered on-site at a new permanent supportive housing complex, linking clients to life skills, health services, counseling, and job training.

The role requires 1-2 years of experience and a BA; reliable transportation; and the ability to work five days a week in the office.

Qualifications

• 1-2 years of experience or related work and BA Degree.
• Experience with youth experiencing homelessness and housing instability.
• Reliable, flexible team player with good work ethic and boundaries.
• Reliable transportation: valid driver’s license and car insurance.
• Proficient in Microsoft Office suite.
• Not a work-from-home position; reports to the office 5 days a week.

Responsibilities

• Conduct initial housing intake and needs assessment with each tenant.
• Develop Individual Service Plans (ISP) to guide goals.
• Address obstacles to housing processes and tenancy.
• Conduct at least two face-to-face tenant contacts monthly.
• Hold quarterly meetings to review progress and plans.
• Coordinate with property management, tenants, and providers.
• Educate clients on Fair Housing, rights, and responsibilities.
• Link clients to services for employment, education, and life skills.
• Assist with rental assistance and move-in/eviction support.
• Document progress and manage client files in DHS CHAMP’s system.
• Suggest and organize workshops via Resident Services.
• Support transitions to mainstream non-supportive housing when ready.
