Job summary

Habitat Health seeks a Functional Wellness and Rehabilitation Educator to lead onboarding and training for PTs, OTs, Activities Coordinators, and PCAs at PACE Centers. You’ll ensure consistency in onboarding, workflows, and educational resources across centers.

Based in California with up to 75% regional travel during launches, you will partner with Practice Operations, Center Leadership, and People Team to optimize programs and ensure high‑quality participant care and regulatory compliance.

Qualifications

• Active California PT or OT license required.
• 3–5+ years of experience teaching or training in the field.
• Experience delivering learning programs and developing workflows or SOPs.

Responsibilities

• Oversee onboarding, practicum and shadowing for PT/OT/Activities Coordinator/PCA roles across centers.
• Lead on-site learning experiences for new center launches.
• Coordinate competency signoffs, skill validation and preceptor checklists.
