Job summary

Neighborhood Healthcare seeks an experienced Rehabilitation Manager to oversee OT/PT/SLP services across PACE programs. You will lead a team of licensed therapists and aides, ensuring high-quality, compliant care for elderly participants in home and facility settings.

You will coordinate with IDT, manage budgets, and drive workforce planning while maintaining regulatory standards. Requires CA licensure, 5+ years of OT/PT leadership, and travel between sites.

Qualifications

• Graduation from an accredited OT or PT program; CA license required.
• Minimum 5 years OT/PT experience; leadership experience preferred.
• Licensed in California; BLS and CPR certifications required (First Aid as applicable).
• Experience with geriatric populations; willing to travel between sites.

Responsibilities

• Orients new staff to PACE model, CMS, and regulatory requirements.
• Provides ongoing coaching, mentoring, and training to promote clinical excellence.
• Leads and supervises therapy professionals across OT/PT/SLP and aides.
• Monitors staffing patterns, caseload distribution, and service coverage for efficiency.
• Oversees DME process and in-home installations to enhance independence.
• Ensures compliance with policies, procedures and regulatory requirements; delivers high-quality care.
• Facilities IDT meetings coordination and multidisciplinary collaboration.
• Oversees SNF discharges and transitions; coordinates with providers for continuity of care.
