Job summary

Habitat Health seeks a Functional Wellness and Rehabilitation Educator to lead onboarding and training for PTs, OTs, Activities Coordinators, and PCAs at PACE Centers. This field-based role oversees learning fidelity across centers and drives standardization of workflows and competency assessments.

The incumbent will partner with Practice Operations, Center Leadership, and People Team to optimize materials, documentation, and processes while traveling regionally to support new center launches in

Qualifications

Active California PT or OT license.
3–5+ years teaching or training in the field.
Proven facilitation and strong communication skills.
Experience delivering learning programs.
Experience developing workflows, SOPs, or training materials.
Ability to coach leaders and build relationships.

Responsibilities

Oversee on-site onboarding and training for PT/OT, Activities Coordinators, and PCA roles.
Lead onboarding, practicum and shadowing across Center launches.
Ensure competency signoffs and skill validation for assigned roles.
Partner with Practice Operations, Center Leadership, People Team on training programs.
Visit centers regionally to support learning needs and drive continuous improvement.
Develop and maintain training materials, SOPs, and role playbooks.
