Job summary

Flex Revive Solutions in Boynton Beach, FL seeks a licensed Physical Therapist for a full-time, onsite home care role. You will conduct patient assessments, develop individualized plans, apply varied therapy modalities, and monitor progress to restore mobility and enhance quality of life.

You will work with two PTs and a PTA in a well-equipped facility, using EMR like WebPT, collaborate with other clinicians, educate patients, and manage a modest caseload to deliver compassionate, evidence-based

Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Active state Physical Therapist License with no disciplinary actions.
• Proficient manual therapy and rehabilitation techniques.
• Familiarity with EMR software such as WebPT.

Responsibilities

• Conduct thorough patient assessments to identify rehabilitation goals.
• Develop individualized treatment plans using multiple modalities.
• Monitor progress and modify care plans as needed.
• Manage a caseload of 2–3 patients per hour.
• Educate patients on exercise programs and lifestyle changes.
• Collaborate with other healthcare professionals to coordinate care.
• Encourage patients to actively participate in recovery.
