Job summary

Senior Care Solutions Group is seeking a compassionate Per Diem Physical Therapist (PT) or Physical Therapist Assistant (PTA) to deliver skilled services in patients' homes in Boca Raton, FL and surrounding territory. You will evaluate patient needs, design individualized treatment plans, and implement evidence-based therapies to improve mobility and function.

The role offers a flexible PRN/part-time schedule, competitive per-visit compensation, and the rewards of one-on-one care in a home

Qualifications

• Florida license as PT or PTA is required.
• Minimum 1 year of licensed PT/PTA experience in home health.
• Familiarity with Medicare documentation requirements.
• Proficiency with EMR systems (Kinnser/WellSky or similar).

Responsibilities

• Perform in-home PT evaluations and reassessments following physician orders and Medicare guidelines.
• Develop and implement individualized therapy plans to restore function and mobility.
• Instruct patients and caregivers on safe use of devices and exercises.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with nurses, therapists, aides, and physicians to ensure coordinated care.
• Maintain accurate documentation in the EMR in a timely manner.
• Travel to patient homes within the assigned territory.
