• Ability to coordinate care and provide guidance related to Home Care needs, including home exercise programs and safety recommendations.
• Current Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
• Active Florida Physical Therapist license (or eligibility to obtain) in good standing.
• Strong documentation, time-management, and organizational skills, with proficiency in using electronic health records.
• Ability to work on-site full time in West Palm Beach, FL, and to support a diverse patient population with professionalism and cultural sensitivity.

Responsibilities

• Evaluate patients, develop individualized treatment plans, and provide evidence-based therapeutic interventions to improve mobility, function, and quality of life.
• Document all patient encounters accurately, educate patients and families on self-management and injury prevention.
