Monadnock Care Solution Physical Therapy is seeking a dedicated Physical Therapist for a full-time, onsite position in Woodbridge, Virginia. You will evaluate patients, design evidence-based plans, and deliver outpatient therapy with manual techniques and functional training.

Qualifications

• Bachelor's degree in Physical Therapy; DPT preferred.
• Valid state Physical Therapy license.
• BLS certification from the American Heart Association.
• Strong communication and computer/EHR skills.
• Passion for building patient relationships and improving function.

Responsibilities

• Conduct thorough evaluations of patients referred by physicians to determine PT needs.
• Create and implement personalized, evidence-based treatment plans.
• Utilize various modalities and exercises to optimize outcomes.
• Provide skilled outpatient therapy including manual techniques and functional training.
• Educate patients and families on plans and self-management strategies.
• Monitor progress and modify plans to align with goals and directives.
• Document care accurately and maintain high levels of customer service.
• Collaborate with a multidisciplinary team to enhance patient care.
• Meet productivity benchmarks and participate in team meetings.
