Willingway in Statesboro, GA seeks a Case Manager-Outpatient to coordinate intake, treatment planning and service delivery within behavioral health programs. You will partner with the treatment team to ensure smooth admissions, discharge planning and accurate documentation in line with standards.

Formación

• High School diploma or equivalent required.
• Bachelor's degree preferred in Human Services, Social Work or related field.
• 1+ year experience in behavioral health preferred.

Responsabilidades

• Serves as primary contact for client intake and service coordination.
• Coordinates with staff to ensure all designated services are received.
• Maintains accurate documentation of care, treatment, or incidents.
• Collaborates with treatment team to help clients meet goals.
• Supports billing/authorization through timely documentation.
• Coordinates transportation and assigns driver-approved staff.
• May co-lead groups and manage supply inventory.
