Shields Health Solutions seeks a Clinical Pharmacy Specialist (Hematology/Oncology) for an onsite role in Baton Rouge, LA. The position emphasizes integrated, patient-centered care within designated outpatient clinics and collaboration with the primary care and oncology teams.

The role focuses on medication optimization, protocol implementation, and evidence-based practice with a Monday–Friday daytime schedule.

Qualifications

• PharmD with active LA pharmacist license.
• Oncology Pharmacy Specialty Certification (BCOP) required within 1 year of hire.
• PGY1 + PGY2 residency in oncology or related field, or 1 year of relevant practice, or 3 years in the field.

Responsibilities

• Conduct comprehensive disease management assessments.
• Collaborate with on-site and hybrid care teams.
• Develop, maintain, and implement treatment and supportive care plans.
• Monitor regimens for therapeutic goals, adverse effects, and drug interactions.
• Ensure compliance with FDA standards and evidence-based literature.
• Identify and resolve medication-related problems to ensure safe pharmacotherapy.
• Provide direct counseling on medication use and disease states.
• Address cost, access, and health literacy barriers to adherence.
