Qualifications

Responsibilities

• Provide comprehensive medication management including reconciliation and dose optimization.
• Collaborate with care teams to order individualized care plans under a Collaborative Practice Agreement.
• Monitor therapy effectiveness and safety, identifying optimization needs and intervening as necessary.
• Support quality improvement and REMS initiatives.

Clinical Pharmacy Specialist, Oncology

Join Shields Health Solutions as an Ambulatory Oncology Clinical Pharmacy Specialist at a nationally recognized Academic Medical Center. You will play a hands‑on role in elevating patient care, managing medication, and collaborating with care teams.

Location: Onsite, Orange, CA

Salary: California employees can expect $150,000‑$190,000 per year (hourly equivalent $72.12‑$91.35), plus bonus according to applicable plan.

Responsibilities

• Provide comprehensive medication management: reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards.
• Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement and promote seamless medication coordination.
• Monitor therapy effectiveness and safety, identify optimization needs, and intervene as appropriate.
• Support quality‑improvement and REMS initiatives.
• Provide recommendations to operations, support teammates with complex prior authorizations, improve workflows, educate patients, and maintain ongoing clinical development.

Qualifications

• Doctor of Pharmacy (PharmD)
• Licensed pharmacist in California
• Meet one of the following:
• Completion of a PGY2 Oncology Residency
• Completion of PGY1 Ambulatory Care Residency and at least 3 years of experience as a clinical oncology pharmacist
• At least 3 years of experience as a clinical pharmacist in oncology, including 1 year practicing under a Collaborative Practice Agreement (CPA)
• Strong communication, organization, and technology skills
