Job summary

Shields Health Solutions seeks a Clinical Pharmacy Specialist focused on Hematology/Oncology. This onsite role in Baton Rouge, LA collaborates with the hospital team to optimize medication therapy, implement protocols, and monitor outcomes.

The ideal candidate will hold a PharmD, Louisiana license, BCOP within 1 year, PGY1/PGY2 or equivalent experience, and demonstrate strong clinical judgment, communication, and risk mitigation in oncology care.

Qualifications

• Doctor of Pharmacy (PharmD) and active Louisiana pharmacist license required.
• Oncology Pharmacy Specialty Certification (BCOP) required within 1 year of hire.
• PGY1 + PGY2 residency in area of specialty/related field or 3 years’ experience in area of specialization.

Responsibilities

• Conduct comprehensive assessments for disease management under collaborative practice agreements or clinic protocols.
• Work on-site in close coordination with interdisciplinary teams, including both in-person and hybrid care models.
• Assist in the development, maintenance, and appropriate use of treatment plans and supportive care plans in alignment with clinical goals and patient needs.
• Monitor and evaluate medication regimens to ensure clinical criteria are met, including therapeutic goals, adverse effects, and drug interactions.
• Identify, resolve, and prevent medication-related problems to ensure safe and effective pharmacotherapy.
• Provide direct, individualized counseling on medication use, lifestyle modifications, and disease state education.
