Job summary

Northwestern Medicine in Chicago seeks a skilled Transplant Nurse Coordinator to support the kidney and liver programs. The role combines clinic care with outreach across satellite clinics and dialysis units throughout the Chicagoland area.

You will coordinate evaluations, educate patients and families, and work with transplant surgeons and nurses to ensure smooth patient journeys from candidacy through post-discharge. Travel is required; bilingual preferred.

Qualifications

• BSN degree is required.
• Illinois RN license required; CPR/BLS certification required.
• Minimum 2–3 years of recent clinical experience; transplant or organ-specific experience preferred.

Responsibilities

• Lead the nurse coordination for kidney and pancreas intake, liver and small bowel intake, or post-transplant, under transplant surgeons and physicians.
• Coordinate patient education, discharge planning, and follow-up communication with families and referring physicians.
• Travel with the kidney transplant team to community satellite clinics and dialysis units to educate patients and professionals about transplant options.
• Participate in multidisciplinary team meetings and quality reviews; maintain accurate medical records.
