Job summary

Northwestern Medicine is seeking a Transplant Nurse Coordinator for the Outreach Kidney Transplant Program in Chicago. The role involves travel across the Chicagoland area to satellite clinics, education of patients and dialysis staff, and coordination of transplant evaluations with the multidisciplinary team.

Requires a BSN, IL RN license, CPR/BLS, and 2–3 years of related clinical experience. This is a full-time, days-position not remote, with travel expected to support outreach and patient

Qualifications

• Bachelor of Science in Nursing (BSN) required.
• Licensed Registered Nurse (RN) in Illinois.
• CPR-Basic Life Support (BLS) certification.
• Minimum 2-3 years of recent & related clinical experience.

Responsibilities

• Travel with the kidney transplant team to community satellite clinics.
• Lead nurse for kidney transplant evaluation at satellite sites.
• Educate patients and professionals about kidney transplant options.
• Coordinate transplant candidate evaluation and living donor processes.
• Support post-transplant inpatient and discharge planning.
