Qualifications

• High school diploma or GED required; advanced education desirable.
• Strong verbal, written, and interpersonal communication skills.
• Proficiency in Microsoft Word, Excel, Outlook, and PowerPoint.

Responsibilities

• Ensures cases are filled within established time period.
• Exhaust all means to reach patient when scheduling communication is needed.
• Assist with referral process as needed.
• Maintain schedule for services requested and provided to patients.
• Respond to emergency calls and arrange schedules accordingly.
• Notify supervisor and CTMs of clinical needs and changes.
• Prepare required reports and documentation timely.
• Ensure patient care coverage and assign appropriate staff per care plan.
