Qualifications

• 1 year of Home Health Scheduling preferred
• Medical Office Experience a plus
• Strong communication and organizational skills
• Ability to work independently
• Valid driver's license and reliable transportation (if applicable)
• Commitment to compassionate, patient-centered care

Responsibilities

• Answers incoming phone calls, gathering and processing patient information from various sources for initial assessment of referrals.
• Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.
