Job summary

Ennoble Care is seeking a Care Coordinator to work on-site at partnered dialysis clinics in Lehigh, PA. You will engage patients, coordinate plans with Ennoble providers and facility staff, and manage visits and follow-ups to ensure timely communication and care continuity.

The role emphasizes compassion, organization and collaboration in a fast-paced clinical environment, with a Monday–Friday schedule and no weekends. Compensation includes hourly pay and benefits after 30 days.

Qualifications

• Active, unrestricted clinical certification or licensure required (PCT, CNA, MA/CMA, LPN, RN or equivalent).
• Dialysis-specific certification preferred (CDT/CHT/CCHT).
• Minimum 1 year experience with hemodialysis/nephrology patients in dialysis settings.
• Strong care coordination knowledge and healthcare terminology.

Responsibilities

• Engage dialysis patients on-site to support care plans and follow-ups.
• Coordinate care using EMR to ensure continuity and timely follow-up.
• Schedule provider visits and support daily clinic workflow.
• Collaborate with providers, dialysis staff, and care teams.
• Communicate patient needs and changes in condition promptly to leadership.
• Maintain accurate documentation per standards and regulations.
• Demonstrate professionalism and strong communication across teams.
• Foster positive relationships with patients, families, and staff.
