Job summary

Anew Care is seeking a Clinical Documentation Coordinator to support our Home Health, Hospice, and Palliative Care teams. This onsite role is based in Indianapolis, IN and focuses on documentation management, regulatory compliance, and workflow coordination to ensure timely completion of records.

You will handle EMR e-fax, DocuSign, and portals for signatures, track outstanding items, and assist with audits and records requests while supporting leadership with process improvements.

Qualifications

• Experience in home health, hospice, or healthcare administration preferred.
• Familiarity with EMR systems and electronic signature platforms.
• Strong follow-through and accountability.
• Ability to track multiple items simultaneously.
• Highly organized and detail-oriented.
• Understanding of Medicare documentation requirements is a plus.

Responsibilities

• Send clinical and administrative documents via EMR e-fax, DocuSign, and healthcare portals for required signatures
• Track outstanding documents and proactively follow up with physicians, facilities, and internal team members
• Maintain organized tracking systems to ensure timely completion
• Escalate delays that may impact billing or compliance
• Process medical records requests and assist with documentation gathering for audits
• Maintain accurate tracking logs of employee-issued equipment and records
• Coordinate with leadership to ensure accountability
• Assist leadership with documentation workflow improvements and identify opportunities to streamline follow-up and tracking processes
