Qualifications

Responsibilities

• Support members in-person and virtually with real-time education and scheduling of referrals and appointments.
• Follow through on appointments to support improved health outcomes and cost savings.
• Be an empathetic problem-solver and a visible partner to patients.
• Clarify referral for specialist and diagnostic service recommendations.
• Care for end-to-end processes for referrals or specialist services as needed.
• Assist in patient outreach and quality programs to support gaps in care and clinical needs.
• Provide education and reminders to members with chronic conditions.
• Lead service and schedule requests on behalf of members.
• Inform and equip Care Team with referral information and requirements.
• Assist in specialist relationship management.
