Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members through critical transitions in acute care settings at the VCU Medical Center in Richmond, VA. The role blends in-person assessments with remote coordination in a hybrid work model.

You will lead discharge planning, identify gaps in care, educate members and families, and collaborate with providers to ensure safe, timely care while maintaining privacy and infection control standards.

Qualifications

• Current, unrestricted RN license in Virginia.
• Must reside within 30–60 minutes driving distance to VCU Medical Center.
• Minimum 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health-plan settings.

Responsibilities

• Perform face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning from admission to post-discharge and facilitate follow-up visits.
• Identify care gaps and barriers; develop and implement strategies.
• Collaborate with providers, facility case managers, and health teams for coordinated care.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately; maintain infection control and privacy standards.
