Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members during critical care moments, delivering face‑to‑face assessments and leading discharge planning at assigned facilities in the Richmond, VA area.

You will identify gaps in care, coordinate with providers and care teams, educate patients and families, and ensure smooth, safe transitions from hospital to home in a hybrid work model that blends site presence with remote documentation.

Qualifications

• Current, unrestricted RN license in Virginia.
• Minimum 3 years acute care nursing experience.
• Experience with discharge planning or care coordination.

Responsibilities

• Conduct comprehensive, face-to-face assessments for newly admitted members.
• Lead discharge planning from admission to post-discharge.
• Identify gaps in care and develop strategies with care teams.
• Collaborate with providers and facility case managers for coordinated care.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately and promptly.
• Maintain infection control and patient privacy standards.
