Job summary

Humana is seeking an Onsite Care Coordination Nurse in Virginia to support members through critical moments of their care journey. You will conduct face-to-face assessments at assigned acute care facilities and lead discharge planning to ensure safe, timely transitions from hospital to home in a hybrid in-person/remote model.

The role requires an active Virginia RN license, at least three years of acute care experience, and ability to coordinate with providers and care teams.

Qualifications

• Current, unrestricted RN license in Virginia.
• Within 30–60 minutes driving distance to VCU Medical Center.
• Minimum 3 years acute care nursing experience.
• Experience with discharge planning or care coordination.
• Proficient in Microsoft Office and electronic medical records.
• Valid driver's license and personal auto insurance.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members in acute care facilities.
• Lead discharge planning and coordinate follow-up with primary care providers.
• Identify gaps in care and implement strategies to address them.
• Collaborate with providers, case managers, and care teams for coordinated care.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
