Responsibilities

• Coordinate discharge planning with PAC/inter-disciplinary teams.
• Engage patients/families to share information and facilitate informed decisions.
• Review SNF assessments and CMS criteria on admission and periodically.
• Document timely and accurate records and maintain HIPAA confidentiality.
• Attend team rounds and ensure appropriate referrals to health plans and community services.
