Job summary

Optum Home & Community Care, part of UnitedHealth Group, seeks a Care Coordinator - Onsite for NaviHealth in Bend, Oregon. You will work with the post-acute care team to coordinate discharge planning, complete weekly functional assessments, and ensure smooth transitions of care for patients moving from hospital to home.

The role requires an active clinical license (RN, PT, OT, or SLP) and at least 2 years of clinical experience, with on-site location within a 30-mile radius.

Qualifications

• Active, unrestricted clinical license in state of hire (RN, PT, OT, or SLP).
• Minimum 2 years of clinical experience.

Responsibilities

• Perform SNF assessments using CMS criteria upon admission and during stays.
• Collaborate with health care teams to plan discharge and transition.
• Engage with patients and families to share information for informed decisions.
• Coordinate care transitions promptly and efficiently across settings.
• Maintain confidentiality of PHI in accordance with HIPAA and regulations.
• Enter timely documentation into coordinate system and update records.
• Attend SNF rounds and provide updates to the Transitions leadership.
